Out-of-competition testing does the real work
Many substances are used during training rather than at competition, and clear from the body before an event. Testing only at competitions would miss them entirely.

Missed tests and filing failures carry sanctions of their own, because a system relying on unannounced collection collapses if athletes can be unavailable without consequence. Repeated failures are treated as a violation in themselves.
Samples are split and stored
Each collection is divided into two sealed portions. One is analysed and the other is retained so an adverse finding can be confirmed independently.
Samples are also stored for years and can be reanalysed later, which matters because detection methods improve faster than the substances themselves change.
Retrospective analysis has reallocated medals long after the event, and the possibility of it functions as a deterrent in its own right.
The biological passport looks for patterns
Rather than seeking a substance, the passport tracks an athlete's own blood and steroid markers over time and flags deviations from that individual baseline.
This addresses methods that use naturally occurring substances or the athlete's own blood, where no foreign compound exists to be detected.
Because it relies on variation over time, the passport needs a long series of samples, and a single unusual reading is treated as a signal to investigate rather than a finding.
Therapeutic exemptions handle legitimate medicine
Some prohibited substances have genuine clinical uses. An exemption process allows their use where documented medical need exists and no permitted alternative is available.
Applications are reviewed by physicians independent of the athlete's team, and approvals are time-limited and specific to the condition and dosage.
The process is criticised from both directions, as a potential loophole and as an obstacle to athletes with real conditions, which reflects the difficulty of the judgement.
Enforcement depends on independence
Testing organised by the same bodies that benefit from clean results has an obvious conflict, which is why independent testing agencies were established.
Laboratory accreditation, chain-of-custody documentation and appeal rights exist because sanctions end careers and must survive legal challenge.
Where national programmes have failed, the failure has usually been institutional rather than scientific, which is why governance oversight now accompanies the laboratory work.
